  1. Separate incidents, access requests, data questions, how-to help, defects, and improvement requests.
  2. Define severity using service impact, affected people, time sensitivity, safety, privacy, and financial exposure.
  3. Name the first responder, resolver, escalation owner, response target, and communication rhythm for each class.
  4. Review repeat demand for documentation, training, configuration, capacity, or vendor-management fixes.
